Please complete free goods or rebate request form at info.coltene.com/promos. To qualify for quarterly promotions, request must be submitted within 30 days of the end of the promotion period. For annual promotions and rebates, request must be submitted within 90 days of purchase or 1.31.27, whichever is sooner. Incomplete submissions or invoices with backordered product will not be processed. Orders must be shipped in full.
